After graduating as Valedictorian of his high school class, Dr. Sam attended Northwestern University, completed his Bachelor's and earned his Doctor of Dental Surgery (DDS) degree also at Northwestern University Dental School. Dr. Sam has received recognition from Consumer's Research Council of America and was presented with the "America's Top Dentists Award" for his extraordinary dental care. He was also presented with the National Leadership Award. Associate Fellow: American Academy of Implantology. Member: American Dental Association (ADA), Chicago Dental Association (CDS), Illinois State Dental Association (ISDS), American Orthodontic Society (AOS) - Continuing Education Member, American Academy of Implant Dentistry Society.
