David’s interest in dentistry grew into a lifelong commitment while working in his family’s practice during summer vacations. His father, Roland Slyby, one of the areas most respected dentists, along with his hygienist mother Connie were excellent role models for David. After graduating from Snider High School, he went on to major in chemistry and minor in psychology at Indiana University. David then earned his doctorate from Indiana University’s School of Dentistry (IUSD) in 1998. While enrolled at IUSD, David was asked to instruct a dental materials class. He thoroughly enjoyed his teaching experience and it would unknowingly play a role in his life again. David returned to Fort Wayne to join Dr. Gerry Kaufman’s practice and established his own satellite office in southwest Fort Wayne. In the fall of 2000, David was offered a teaching position in the dental hygiene department at IPFW. He taught for four semesters and continues to participate in high school and state college career internship programs. In 2008, he was honored to be selected by Orthopedics Northeast as the primary care dentist for the areas collegiate athletes. He is currently a member of the Indiana Dental Association and the American Dental Association as well as the Fort Wayne chapter of the Isaac Knapp Dental Society. Since Dr. Kaufman’s retirement in 2002, David has kept his commitment to his patients by ensuring that the practice stays at the forefront of dental care. While carefully evaluating and incorporating new techniques and technology, David’s also made sure that his eight-member staff is fully certified and – equally important – sensitive to the individual needs of each patient.
