Dr. Kowalik received his Bachelor of Science, 1969, and Doctor of Dental Surgery, 1973, from Loyola University. He taught at Loyola Dental School in oral diagnosis and prosthetics. Served on committees for Illinois State Dental Society and Chicago Dental Society, president and board member of dental organisations and Burbank Chamber of Commerce. Member American Dental Association, Illinois State Dental Society, Chicago Dental Society. Over three thousand hours continuing education. Completed IV sedation training at University of Illinois Medical Center. Lives in Riverside with wife Linda, enjoys bicycling, woodworking, gardening, hiking National Parks.
